Strength assessment of endodontically-treated teeth based on a 3D numerical model

Abstract

Introduction. Knowledge of the biomechanical behaviour of endodontically-treated teeth underloading in the oral cavity should be included in the clinical proceedings. Because it is difficult to analyze stress distributions in vivo, computer methods are increasingly being used. Aim of the study. To perform a comparative numerical analysis of stresses and strains in healthy and endodontically-treated teeth with the canal filled with gutta-percha or resilon. Materials and methods. The study was carried out on a 3D model of the upper incisor with an extremely well-preserved crown (with only an endodontic access opening) after virtual simulation of canal preparation with 2% taper instruments and gutta-percha or resilon filling at 2 mm from the root apex. The model was exposed to a load of 100 N corresponding to the occlusal situation of the incisors in centric occlusion. To determine the distribution of stresses and strains, the finite element analysis was used. Results. The maps of stress and strain distributions in all three analyzed models were very similar to each other. Increased stress concentrations was found cervically and at the middle of the root (mainly on the labial side). Stresses grew from the root apex towards the crown, with the highest value at the outer surface of the tooth, and were the lowest inside the canal. Strains ε were minimal and reached values of 10-4. Conclusion. Within the limitations of the study, it can be concluded that the physiological load of the incisor with wellpreserved crown after endodontic treatment causes stress and strains similar to a healthy tooth. Filling the canal with either gutta-percha or resilon results in comparable stress distribution. An indication of the most strained areas, which are the cervical region and the middle parts of the root, is of clinical value.
