Streptokinase Response and its Adverse Drug Reactions in Acute Myocardial Infarction Among Different Age Groups at Sudan Heart Center, Khartoum-Sudan

Abstract

Thrombolytic agents are among the medications that are used widely for the treatment of Thromboembolic disorders and myocardial infarction (MI). Despite the world-wide availability of newer specific agents, streptokinase (SK) is still the most frequently used medication from this class in Sudan. Hence we conducted this study to assess the response to streptokinase and its adverse drug reactions in acute (MI) among different age groups. We performed retrospective hospital-based cross-sectional study using designated data collection sheet, using convenient sample; between January 2015 to August 2016 at Sudan Heart Center. And found that 170 cases during a study period .100 cases remained after implementing inclusion and exclusion criteria. The most frequent adverse drug reactions (ADRs) were related to the cardiovascular system. Among them hypotension and arrhythmia were the most frequent in both gender 29% and 7% respectively. The second most prevalent ADRs were fever and others (cardiac arrest and loss of consciousness) each 6%. Also hematological, gastrointestinal, hypersensitivity, renal and neurological were reported. Patients under study had high response to standard regimen at all age period, especially elderly group (60-69 years and >70 years), diabetic patients and patients receiving aspirin concomitantly with (SK) were responded (p-value < 0.05); even coming in first 12 to 24 hours (p-value > 0.05). Also all patients who received (SK) in the last four years can receive new dose safely (p-value > 0.05).We conclude that the majority of these ADRs were similar to previous studies, the response to standard regimen was obvious among all age groups and all discussed associated factors were contributed as appositive value for drug response.
