Study of Autonomic Manifestations in Patients with Diabetes Mellitus Attending a Tertiary Care Hospital in Coastal City of South India

Abstract
Abstract: Diabetes Mellitus, in all its forms has emerged as a major health burden worldwide including India. Autonomic neuropathy is one which is often a disabling complication of Diabetes Mellitus. 50 patients admitted with Diabetes Mellitus, both Type 1 and Type 2, under Medicine department, Kanachur Institute of Medical Sciences, from December 2016 to May 2017 were studied. A series of bedside tests, evaluating the parasympathetic and sympathetic functions were conducted on all the study members to see for dysautonomia. The maximum age of the patients studied varied from between 51-60 years. Majority of patients were males (68%), 92 % of the cases had at least one symptom of dysautonomia, Impotence was the commonest symptom, (36%) ,followed by postural giddiness (30%). 28 of them had positive Autonomic scores (56%) and among the 10 Type 1 Diabetics in the cases, 5 had positive autonomic scores (50%). Polyneuropathy was the commonest complication among the cases (44%), Mean Glycosylated Haemoglobin (HbA1c) was 8.24±2.59%. 4 out of the 10 Type 1 Diabetics had prolonged corrected QT-interval (40%). Among the cases the corrected QT-interval was 6.54±0.84 seconds for those with positive autonomic scores than 1.23±1.34 seconds in those with negative scores.
