Study Of Pseudocholinesterase In Organophosphorous Poisoning At A Tertiary Care Hospital, MGM, Kakatiya Medical College, Warangal, Telengana

Abstract

Background: Organophosphorous (OP) poisoning is a common public health problem in many developing countries. Organic insecticides are commonly used in agricultural sector. These organic insecticides are abused for suicidal purpose in rural areas of the country. Easy accessibility, low cost and because of its quick action, rural population select this method. OP acts by inhibiting the enzyme cholinesterase, resulting in accumulation of acetylcholine at synapses and myoneural junction leading to cholinergic over activity. The diagnosis and severity of acute OP poisoning is suggested by a depressed serum butyrylcholinesterase or red blood cells cholinesterase level. Estimate the level of pseudocholinesterase as a diagnostic and prognostic marker of organophosphorus poisoning. Materials And Methods: The study is a hospital based study for one year from August 2013 to September 2014. It was conducted in a medical emergency ward of tertiary care hospital located at Warangal attached to Kakatiya Medical College. The study subjects were patients getting admitted with OP poisoning in the age group of above 18 taken criteria to recruit in the study. Patients below 18 years, chronic hepatic disease sufferers and pregnant were excluded for the study. The results were analyzed by using SPSS software. Observations And Discussion: The patients were graded as per Pseudocholinesterase levels in to 4 groups. 5 ml of venous blood collected in plain tubes and subjected to 3000 rpm for 10 minutes. Serum separated 2 hours after collection of blood. Serum Pseudocholinesterase level noted initially before treatment on day 1 and 48hrs after treatment. New German Society for Clinical Chemistry (DGKC) method adopted to estimate enzyme levels. More than 50% of study subjects are in the age group of 18-30 years. Victims are male predominant. Mean values of cholinesterase were low in cases compared to controls. Conclusion: OP poisoning is diagnosed by history of symptoms and signs and by estimation of pseudo cholinesterase (PChE) enzyme which is present in serum and is closely resembles RBC acetyl cholinesterase present at the nerve endings which is responsible for the breakdown of acetylcholine.
