Study Of Variations In Anatomy And Intramedullary Canal Diameter Of Clavicular Bone In Indian Population

Abstract
Background and objective Although the length and shape of the clavicle are well understood, the dimensions of the medullary canal although available for other long bones have not been clearly defined for clavicle. The variability of the curvatures of the clavicle are also not well understood among the Indian population.These parameters have great importance when considering intramedullary nailing techniques. Materials and methods 100 cadaveric clavicles of both sexes were analysed. Results The mean medial end diameter in male right clavicles in Antero posterior and Supero inferior planes were 17mm and 16.5mm respectively. In male left clavicles it was 15.5mm and 16.25mm respectively. In female right clavicles it was13.9mm and12.6mm respectively.In female left clavicles it was 14.75mm and 12.7mm respectively. The mean middle 3rd diameter in male right clavicles in AP and SI planes are 7.5mm and 5.8mm respectively. In male left clavicles it was 6.25mm and 6mm respectively. In female right clavicles it was 5.5mm and 5.7mm respectively.In female left clavicles it was 5mm and 4.6mm respectively. The mean isthmus medullary canal diameter in male right clavicles in AP and SI planes were 5.1mm and 5.6mm respectively. In male left clavicles it was 4.45mm and 5.9mm respectively. In female right clavicles it was 4.25mm and 4.8mm respectively. In female left clavicles it was 4.25mm and 4.75mm respectively.The overall average of male and female clavicles in AP plane was 4.45 mm. The mean lateral end medullary canal diameter in male right clavicles in AP and SI planes were 7.1mm and 19.4mm respectively. In male left clavicles it was 5.9mm and 17mm respectively. In female right clavicles it was 5.45mm and 16.9mm respectively. In female left clavicles it was 4.9mm and 15.25mm respectively. Conclusion The narrow point is the location of isthmus in majority of the clavicle fractures, which adds considerable difficulties for intramedullary fixation.The AP diameter at the isthmus is the narrowest part of the medullary canal and this has been not addressed in any literature to our knowledge. This gains importance in the Intramedullary nailing and future nail designs for clavicle fracture fixation.
