Study on the effectiveness of fenofibrate combined with low-molecular-weight heparin calcium in the treatment of hyperlipidemic acute pancreatitis

Abstract
Objective To study the effectiveness of fenofibrate combined with low-molecular-weight heparin calcium in the treatment of hyperlipidemic acute pancreatitis ( HLAP) . Methods Sixty-eight patients with mild HLAP admitted from May 2019 to May 2021 were divided into experimental group ( n = 34) and control group ( n = 34). After receiving condition assessment, the control group was treated with comprehensive therapeutic measures, including standardized treatments such as fasting and no alcohol use, gastrointestinal decompression, acid suppression, inhibition of pancreatic enzyme secretion, analgesia, blood volume supplementation, maintenance of water-electrolyte and acid-base balance, and strict diet control, and subcutaneous injection of 0. 5 mL of low-molecular-weight heparin calcium every 12 h for 7 d. On the basis of the treatment of the control group, the experimental group was added with fenofibrate capsule 0. 2 g on the second day of admission, once a day. Both groups were treated continuously for 7 d as a course of treatment, and indicators were detected and clinical efficacy was analyzed on the day after the course of treatment. Results After 7 d of treatment, the neutrophil ratio, C-reactive protein, amylase, lipase, aspartate aminotransferase, lactate dehydrogenase, blood calcium, urea, and triglycerides levels in the experimental group and control group changed significantly compared with those before treatment (P < 0. 05). Inflammatory factors and most serological indicators decreased significantly in both groups, and blood calcium levels increased compared with those before treatment ( P < 0. 05). C-reactive protein, blood amylase, aspartate aminotransferase, lactate dehydrogenase, blood calcium, triglycerides, tumor necrosis factor-α and interleukin-6 in the experimental group were significantly different from those in the control group (P < 0. 05). The total effective rates of the experimental group and control group were 97. 12% and 70. 59% , and the difference between the two groups was statistically significant (P < 0. 05). Meanwhile, the experimental group had significantly shorter remission time of abdominal pain and distension, time to first bowel movement and total hospital stay ( P < 0. 05 ). Conclusion Fenofibrate combined with low-molecular-weight heparin has a significant effect in the treatment of mild HLAP, which can fundamentally eliminate the risk factors of the disease, increase the blood calcium concentration and improve the prognosis of patients with pancreatitis.
