Surgical Management of Single and Multiple Brain Metastases

Abstract

The aim of this study was to assess importance of the surgery among the treatment modalities for patients with brain metastasis (BM). Retrospective review of 48 BM patients newly diagnosed at our neurosurgery clinic between January 2001 and January 2005. The median patient age was 52 years . Thirty-nine patients had single and nine patients had multiple cerebral metastatic lesions. MRI of brain revealed lesions in the cerebrum in thirty-two, in the cerebellum in nine, cerebrum and cerebellum in seven and in the brain stem in one. Among 48 patients, primary malignancy was determined in 29 before surgery. Nineteen patients presented with symptoms of intracranial mass lesions was not known systemic cancer history We performed total excision of lesion in 47 patients and subtotal excision of lesion in one patient. Neurological deterioration did not occur after surgical procedures. Primary neoplasm of the metastasis were lung Ca in 30, breast Ca in 3, melanoma in 4, colon Ce in 2, testis Ca in 1, renal Ca in 2, prostate Ca in 1, thyroid in 1 and synovial sarcoma in 1. We could not determine the primary neoplasm in three, although the histological confirmation was metastasis. All patients with diagnosed metastatic diseases received whole brain radiotherapy (WBRT) after surgery. Our study results demonstrated that the cranial metastasis patients with high ICP-related symptoms or major neurological deficit with acute onset useful surgery.
