The assessment of serum C-reactive protein, interleukin 6 and its soluble receptor in children with non-alcoholic fatty liver disease

Abstract
<b> Objectives.</b>Nonalcoholic fatty liver disease (NAFLD) is an inflammatory disorded, characterized by low-grade systemic inflammation. C-reactive protein (CRP) and interleukin 6 (IL-6) are regarded as markers of inflammation. Therefore, the aim of the study was to evaluate the serum levels of hsCRP, IL-6 and its soluble receptor (IL-6sR) in obese children with NAFLD.<br><b> Material and methods. </b> Fasting serum levels of hsCRP, IL-6 and IL-6sR were determined (nephelometry, and ELISA respectively) in 45 consecutive obese children (BMI>97 pc, aged 7-17 years, mean age 12 years) with suspected liver disease (hepatomegaly and/or elevation of serum transaminases activity and/or liver steatosis in ultrasound). Viral hepatitis (HBV, HCV), metabolic liver diseases (Wilson disease, alpha 1 antitrypsin deficiency) and autoimmune hepatitis were excluded in all children. The degree of liver steatosis was graded in ultrasound according to Saverymuttu. <sup>1</sup>HMR spectroscopy was performed with 1.5 T scanner with PRESS sequency.<br><b> Results. </b> A fatty liver was confirmed in 32 children by ultrasonography (16 of them also had increased ALT activity). hsCRP level correlated with GPT (r=0.44, p=0.012) and GGT activity (r=0.37, p=0.04). The ability of hsCRP to differentiate children with liver steatosis diagnosed in <sup>1</sup>HMRS from those without steatosis was significant (AUC=0.697, p=0.04). We did not find such correlations for IL-6 and IL-6sR.<br><b> Conclusion. </b> hsCRP may be regarded as a suitable serum marker predicting liver steatosis in obese children.
