The correlations between the clinical and anamnestic characteristics and biochemical parameters of bone tissue metabolism at the comorbidity of chronic pancreatitis and arterial hypertension

Abstract

Objective — to establish the presence of correlations between the clinical and anamnestic characteristics of patients and biochemical indicators of bone tissue metabolism, as well as the presence of fractures in patients with a combined course of chronic pancreatitis (CP) and arterial hypertension (AH). Materials and methods. The examinations involved 110 patients with CP, among them 40 patients had an isolated CP course, and 70 subjects had concomitant AH. The diagnostic search included clinical and anamnestic characteristics of patients (gender, age, disease duration, level of pancreatic elastase 1) and biochemical parameters of bone tissue metabolism (osteocalcin, total acid phosphatase (TAP) and tartrate-resistant acid phosphatase (TRAP)), as well as fractures with the determination of correlations between these indicators in patients with CP and AH and the formation of complications. The control group consisted of 78 gender- and age-matching healthy individuals. Results and discussion. Determining the links between the clinical and anamnestic characteristics of patients and the biochemical parameters of bone tissue metabolism allowed to reveal the high (R = 0.60) significant (p < 0.01) correlation between the osteocalcin levels and the level of pancreatic elastase in the group with isolated CP. Moreover, the negative significant (p < 0.01) correlation of the average strength (R = –0.49) between the TRAP levels and patient’s age has been established. In patients with comorbid CP and AH pathology, the relationship of moderate strength between the TRAP and the AH duration was significant (R = 0.36, p < 0.01). The levels of TAP and TRAP exceeded the normal values in individuals of the main group in 2.5 and 1.9 times (KMU; U = 866.0; p < 0.01), and in the comparison group in 2.0 (TAP) and 1.3 (TRAP) times, respectively (KMU; U = 821.0; p < 0.01), testifying the development of osteopenic states. Conclusions. The formation and course of CP, and especially its combination with hypertension, contributes to the early disruption of the structural and functional state of the bone tissue, which is confirmed by the frequent occurrence of limb fractures. In patients with CP, there is a positive correlation between the levels of osteocalcin and pancreatic elastase 1, indicating the relationship between the impaired exocrine pancreatic function and disorders of bone metabolism. An increase in the activity TAP and TRAP takes place both at the isolated course of CP, and in comorbidity of CP and AH, it can be considered as a marker of the formation of osteopenia syndrome.
