The evaluation of specific changes and diagnostic informativeness of the cytokine profile in young patients with gastroesophageal reflux disease, occurring against the background of autoimmune inflammation

Abstract

Objective — to determine the general trend of changes in the indicators of systemic inflammation — interleukin-1β and interleukin-18, in young patients with gastroesophageal reflux disease (GERD) and autoimmune thyroiditis. Materials and methods. The study involved 83 young patients (students aged 18 to 25 years) with GERD and autoimmune thyroiditis (AIT), in whom the levels and role of interleukin-1β (IL-1β) and interleukin-18 (IL-18) have been evaluated. The comparison group consisted of 30 patients with isolated GERD of the same age, gender, and the disease form. Results. It has been established that GERD was accompanied by a significant increase in the IL-1β levels; in association with AIT this indicator increased in 1.7 times. The IL-1β value depended on the form of the esophageal mucosa lesion and was more pronounced in the presence of erosions but did not correlate with their number. In subjects with combined GERD and AIT course, the IL-18 levels increased up to 1761.5 (1451.7; 2876.9) pg/ml, that exceeded this indicator in patients with isolated GERD, 614.6 (521.9; 721.8) pg/ml and controls’ values (p < 0.05). Conclusions. In patients with combined GERD and AIT, the increase of IL-18 levels was observed, that was associated with the inflammatory process in both organs and an autoimmune process in the thyroid gland. Further disease progression may occur due to the preservation of the autoimmune component. The GERD course, especially in case of its combination with AIT in young people, was accompanied by an increase in the peripheral blood levels of interleukin-1β, which indicates the presence of an active inflammatory process.
