The outcome in upper gastrointestinal bleeding

Abstract

Background and Objectives: The upper gastrointestinal bleeding (UGIB) is one of the most important emergency cases that remain the common cause of emergency hospitalization and death occurrence. The aim of this study is to determine the underlying causes of upper GI bleeding in AL-Kadhmiyia teaching hospital and factors influence the outcome results in regards to emergency surgery; spontaneous resolution episodes &mortality incidence. Methods: This is a prospective study of (75) patients presented with upper gastrointestinal bleeding (UGIB) for 18 months from June 2008 to November 2009. All patients underwent endoscopy during the first (48) hours of admission. Results: The patient's age group is between 15-80 years. The male patients 58(77.3%) the female patients 17(22.6%). Duodenal ulcer was the commonest causes of UGIB 35patients (46.6%) followed by esophageal varices 18patients (24%) then gastric erosion 13patients (17.3%) then CA-stomach 4patients (5.3%) then esophageal erosion 3patients (4%) then gastric ulcer 1patients (1.3%). Sixty patients (80%) of those patients responded to conservative treatment while 15 patients (20%) required surgical treatment, as emergency operation in 10 patients (13.3%) or elective operation in 5 patients (6.66%). The highest mortality rate was found patients with hypotension 9 patients (12%) then in those patients with age more than 50 years 6 patients (8%). Conclusions: The commonest cause of UGIB in AL-Kadhmiyia teaching hospital is chronic duodenal ulcer.
