The role of autologous hematopoietic stem cell transplantation in the treatment of multiple sclerosis

Abstract

Multiple sclerosis is a chronic disease of the central nervous system with inflammatory and neurodegenerative component and represents one of the leading causes of neurologic disability especially in young adults. Typical feature of multiple sclerosisis the presence within the structures of the central nervous system demyelinating plaques caused by autoimmune process with the involvement mainly of autoreactive T cells. Standard treatment strategies for multiple sclerosis are based on immunomodulatory drugs that affect the regulation of aberrant cell activity, as well as impaired balance in the production of pro- and anti-inflammatory cytokines. However, the use of interferon-β, glatiramer acetate as the first-line treatment, natalizumab and fingolimod as second-line one, has shown limited efficacy in patients with either a progressive or an aggressive disease course. There is growing evidence that autologous hematopoietic stem cell transplantation may be an alternative treatment modality for those patients. It is believed that through that procedure a significant amount of autoreactive lymphocytes is destroyed and by restoring the immune system of the patient organism is more resistant to the development of autoimmunity. This work summarizes recent views on the role of autologous hematopoietic stem cell transplantation in multiple sclerosis.
