The state and ways of improvement of health care for patients with cerebral stroke in Kyiv

Abstract

Objective — to improve the quality of medical care for patients with cerebral stroke. Methods and subjects. The study included an examination of 1575 patients with acute cerebral stroke (males 42.7 %) aged 26 to 100 years old (mean age 70.6 ± 10.7 years) who were hospitalized from June 1 to December 31, 2016 to neurological departments in the city of Kyiv. Results. It has been established that in 71.5 % of the examined patients, stroke occurred for the first time in their life. Patients aged 60 years and older had dominated (84.1 %). The most important factor of vascular risk was hypertension, which was observed in 95.4 % of the patients. 41.6 % of patients sought medical help within the first 3 hours, 32.6 % of patients were hospitalized within the therapeutic window. 86 % of patients were taken to hospital by ambulance, 6 % of patients sought medical help on their own. The most important factor of late hospitalization was late sought medical help, which was observed in 76 % of the patients. Neuroimaging was performed in 1120 (81.2 %) patients. Late hospitalization of patients, the lack of 24­hour neuroimaging at public hospitals in the city, and the serious condition of patients with the presence of decompensated comorbid diseases are the most frequent reasons for the insufficient use of thrombolytic therapy in patients with ischemic stroke in Kyiv. Conclusions. It is necessary to teach population about risk factors and symptoms of stroke and the need to seek immediate medical care in case of the first symptoms of the disease. Urgent medical care for patients with stroke in Kyiv requires significant improvement and organization of stroke units. Currently, the neurological departments in Kyiv have real preconditions for thrombolytic therapy in patients with acute ischemic stroke.
