The state of the proteolytic system in patients with pulmonary tuberculosis

Abstract

Based on literature data, the authors outlined the state of the problem of tuberculosis in Ukraine. The urgency of the problem in their opinion is the late timing of the diagnosis of pulmonary tuberculosis and the complexity of its treatment in view of the increasing prevalence of MDR tuberculosis and XDR tuberculosis. Objective — to establish the dependence of the state of the proteolytic system in paients with pulmonary tuberculosis depending on the sensitivity of mycobacterium tuberculosis to antituberculosis drugs. Materials and methods. Were examined 66 patients with pulmonary tuberculosis. In the first group were 39 (59.1 %) patients with tuberculosis, sensitive to anti-TB drugs, in the second group — 27 (40.9 %) patients with MDR TB. The state of the proteolytic system is studied on the example of neutrophil elastase. Results and discussion. The erythrocyte sedimentation rate was (32.2 ± 2.7) mm/h in patients with sensitive tuberculosis and (23.6 ± 3.3) mm/h in patients with MDR TB, which is more than 1.4 times (p < 0.05). It may be the result of inhibited reactivity in patients with multi-resistant forms of pulmonary tuberculosis. The level of neutrophils in patients of I group was 4.0 ⋅ 109/l, in patients with the ІІ group — 3.7 ⋅ 109/l (p > 0.05). The value of neutrophil elastase in patients with susceptible tuberculosis significantly exceeded the norm by 1.5 times (p < 0.01). In patients with MDR TB the level did not exceed the normal values 1.1 times (p > 0.05), which was statistically not reliable. The level of neutrophil elastase in patients with sensitive pulmonary tuberculosis was significantly higher in 1.4 times than patients with multidrug-resistant pulmonary tuberculosis (p < 0.05), which according to authors suggests inhibition of the reac­tivity of the organism. Conclusions. The authors believe that a higher level of neutrophilic elastase is evidenced by the active lung tuberculosis detected in a timely manner, and less — about the late MDR TB of the lungs.
