The use of the asymmetric appliance with Hyrax screw for lateral crossbite and lack of space for maxillary canine correction – case report

Abstract

The aim of the study was to present the use of the asymmetric appliance with Hyrax screw in the treatment of lateral left crossbite and the lack of space for the right canine in maxilla. The case presented is a 12,3-year-old male, who sought orthodontist opinion due to upper canine vestibular position discomfort. Extra oral examination revealed the following: lower face segment asymmetry with point Pg displacement to the left side, the same side upper lip collapse and profile convexity. Intraoral examination revealed the following: lateral left crossbite, anterior crossbite on lateral incisor, the lack of space for right maxillary canine, the decreased overbite and midlines discrepancy with right side upper line and left side lower line displacement. Cephalometric analysis revealed skeletal class I and orthognatic face type with high angle vertical skeletal base relation. The treatment plan covered Hyrax screw appliance with acrylic pads as the first step, and subsequently upper and lower fixed orthodontic appliances. Expander modification consisted of asymmetric pads covering more teeth on lateral crossbite side, which allowed to obtain wider dental arch expansion on the right side. That provided the patient with the possibility to maintain the mandible symmetrical, thus eliminating lower face segment asymmetry. Dentoalveolar expansion also enabled to regain the space for tooth 13 and its proper position in alveolar process. At the same time, upper and lower midlines and overbite correction improved the function of masticatory system as well as the smile esthetics. (Orthod. Forum 2011; 1: 41 - 49).
