Type 2 Diabetes (T2D) and Arsenic at low concentrations: are there any real associations? A systematic review.

Abstract

Incidence and prevalence of Type 2 diabetes (T2D) are rapidly increasing worldwide, characterizing a true epidemic. Although much is already known about this disease, classical theories on its pathogenesis don’t fully explain the present situation. Current scientific research has focused on environmental factors to fill these knowledge gaps. Long-life exposure to metals, in particular Arsenic, has been identified as a risk factor of T2D. If the association is statistically significant at high levels of exposure, long-term effects of low to moderate Arsenic concentrations remain substantially controversial. The purpose of this systematic review is to shed light on this grey area, focusing on the current knowledge of exposures lower than 100μg/L. The vast majority of reports in the medical literature, published from 2005 to 2016, show a similar trend: they identify a significant association between type 2 Diabetes and Arsenic exposure. Some controversial points still remain, making further research necessary: the poor robustness of most studies, as well as the existence of complex factors that could modify the effect of exposure to arsenic, and the existence of distinct subpopulations, not taken into consideration as heterogeneous in the medical literature. In summary, all these factors should be considered before designing future studies.
