Ultrasound thyroid imaging, thyroid stimulating hormone and anti-thyroid peroxidase antibodies concentrations in adolescents with a family history of thyroid diseases – preliminary results

Abstract

Introduction. Data regarding the thyroid gland evaluation in adolescents with thyroid disease in the family are limited.Aim. The aim of the study was to asses thyrotropin (TSH) and anti-thyroid peroxidase antibodies (anti-TPO) concentrations as well as thyroid morphology on ultrasound in adolescents with a family history of thyroid diseases.Material and methods. The study included 204 patients (129 girls and 75 boys) aged 17-18 years, who declared the presence of thyroid disease in the family and no personal history of thyroid disfunction.Results. The mean thyroid volume was 9.85 ml (4.54-24.69; SD ± 3.31) for female and 13.31 ml (5.46-60.95; SD ± 6.89) for male. Abnormal ultrasound thyroid image was found in 30% of adolescents. The most common pathology was generalized thyroid hypoechogenicity (16.7% of examined students). Cysts were found in 7.4%, and thyroid nodules – in 8.8% of patients. One person had TSH low concentration and 4 person – above normal range. Elevated anti-TPO concentration was found in 6,4% of examined persons. Increased levels of anti-TPO, thyroid hypoechogenicity and focal lesions were significantly more common in girls than in boys. Abnormal ultrasound image was more sensitive indicator of hypothyroidism than elevated levels of anti-TPO antibodies.Conclusions. The results indicate that thyroid ultrasound should be the first-line screening test in adolescents with thyroid disease in the family.
