Urinary tract infections caused by Klebsiella pneumoniae early after kidney transplantation

Abstract
Introduction. Urinary tract infections (UTI) are common complications in patients after kidney transplantation (KTx). Klebsiella pneumoniae is one of the most commonly identified pathogens resistant to many antibiotics.Aim. The aim of the study was to assess the factors predisposing to the infection and its influence on graft function in the first year after KTx.Material and methods. The study group included 38 patients after kidney transplantation with positive urine culture for Klebsiella pneumoniae in the first month after transplantation. 18 patients the pairs to 18 patients from the study group, who received a kidney from the same donor served as controls. Graft function was assessed as the glomerular filtration rate (eGFR) calculated with MDRD formula and presence of urinary abnormalities such as leukocyturia, erytrocyturia and proteinuria. An immunosuppressive regimen and comorbidities were also taken into consideration.Results. No significant difference between eGFR in K. pneumoniae infected KTx patients and those without UTI was found. Mean proteinuria in the study group immediately after KTx and 1, 3 and 6 months after KTx was 0.59, 0.2, 0.15 and 0.14 g/l, respectively and 0.26, 0.27, 0.5, 0.5 g/l in the control group (ns). Other urinary abnormalities were equally prevalent in both groups. 47.7% of K. pneumoniae infected patients were diabetic compared to 22.2% in the reference group (p = 0.087).Conclusions. Klebsiella pneumoniae infection diagnosed in the first month after transplantation seems not to affect graft function in one-year assessment. Diabetes mellitus may be a risk factor of UTIs caused by K. pneumoniae in kidney transplanted patients.
