Use of Closed suction drainage Vs No Drain usage in Orthopaedic surgery- A Prospective Comparative Study

Abstract

Background: Draining the wound has been followed since the ages with no proved benefits. Objective: To assess the effectiveness of suction drain in Orthopedic surgery and see its effectiveness. Methods:112 patients ,56 in each group, Control group (Closed suction drain used) and Study group (No drain used) were studied between Dec 2014 and May 2015.The inclusion criteria was fresh fractures, total knee and hip arthroplasty, Non-union of long bones. The exclusion criteria was Infected Nonunions, Revision arthroplasty surgeries, Compound fractures. Results: No significant difference was seen in age (independent t test, p value 0.2857), Sex (Chi square test-0.6095 df =1), Pre-operative Haemoglobin (Independent t test 0.9381), Post-operative Haemoglobin (Independent t test 0.9702), Pain scores were (study group- 8.14+/-0.92, control group- 8.19+/-0.90)at 6 hours,12 hours (study group- 6.25+/-0.91, control group- 6.28+/-0.90) and 24 hours (study group- 3.98+/-0.92, control group- 3.98+/-0.96) Not significant using students t test. The average duration of hospital stay was 6.27+/-2.20 in study group and 4.5+/-1.29 in control group which was statistically significant (p<0.001). The most common indication was osteoarthritis knee (44.6%) and most common procedure was plating (43.75%). Ecchymosis and change of dressing was more common in study group (Not significant).Conclusion: The practice of draining the wound has no benefits in terms post-operative infection and haematoma formation which is thought to be the main reason for its usage.
