Usefullness of mannheim peritonitis index scoring system in predicting the outcome in patients of periotonitis due to hollow viscous perforation

Abstract

Aims and Objectives: Early prognostic evaluation of patients with peritonitis is desirable to select high- risk patients for intensive management and also to provide a reliable objective classification of severity and operative risk. This study attempts to evaluate the use of scoring systems such as Mannheim Peritonitis Index (MPI), Acute Physiological and Chronic Health Evaluation Score (APACHE) and Sepsis Score which determine the outcome of the disease. Material and Methods: A prospective study was conducted using 100 consecutive patients (66 males, 34 females) having generalized peritonitis over a two- year period. All the three scoring systems were applied to patients before laparotomy. Based upon the scores, patients were arranged into 3 groups in MPI and APACHE and 2 in Sepsis Score. The outcome of the patients was noted and the accuracy of the three systems was evaluated. Results and Discussion: In the MPI system, mortality was zero in the group of patients with a score of less than 15, while it was 4% in the patients scoring 16-25 and 72.72% in those with scores >25. In the APACHE II system, no mortality was noted in patients with scores <10. Mortality was 66.67% and 83.33% in the groups scoring 10-20 and >20, respectively. Similarly, in Sepsis Score 2.2% mortality was noted in patients with score <20 and 69.2% in score >20. Conclusion: All the three scoring systems are accurate in predicting mortality; however, the MPI score has definitive advantages and is therefore more useful.
