VAP-1 in chronic kidney disease and diabetic kidney disease

Abstract

VAP-1 (vascular adhesion protein-1) is a copper-containing SSAO (semi-carbazide sensitive amine oxidase) with dual function. It is secreted by vascular smooth muscle cells, adipocytes, and endothelial cells with functional monoamine oxidase activity. VAP-1 is elevated in hyperglycemia and chronic kidney disease.<br>The aim of the study was to assess VAP-1 levels and its correlations with renalase,which possesses possible monoamine oxidase activity, in patients on peritoneal dialyses in regard to the presence of diabetes.<br>We assessed adhesion moleculeas: P and E-selectins, ICAM i VCAM, CD40L,CD44, CD146 and some hemostatic parameters thrombin-antithrombin comlpexes-TAT, prothrombin fragments 1+2, plasmin-antiplasmin complexes PAP, tissue plasminogen activator-tPA and its inhibitor-PAI-1, VAP-1 and renalase with commercially available assays. Diabetic patients had higher VAP-1 and CD146 levels but lower renalase than non-diabetic subjects. In univariate analysis VAP-1 was related to time on dialyses, presence of diabetes, platelet count, CD146, and renalase was related to presence of diabetes, CD40L and PAP. In multivariate analysis, predictor of VAP-1 was only the presence of diabetes.<br>Concluding, VAP-1, elevated in patients on peritoneal dialyses was predominantly dependent the presence of diabetes, both factors associated with endothelial damage and promoting cardiovascular complications. Taking into consideration the principle of peritoneal dialysis, elevated VAP-1 and markers of endothelial cell damage might contribute to the enhanced progression of atherosclerosis in this population.
