A study to analyse the association between hypothyroidism and gall stone disease

Abstract

Background: Thyroid disorder is a prevalent condition among adult population; however, it is frequently over looked. Hypothyroidism causes low bile flow and sphincter of Oddi dysfunction and hence promotes gall stone formation. This study was done to substantiate the need for evaluating the thyroid status in patients presenting with cholelithiasis/ choledocholithiasis Aim: To study the prevalence of hypothyroidism in patients presenting with cholelithiasis/ choledhocolithiasis. To assess if thyroid profile is indicated in patients with biliary lithiasis. Materials and Methods: This is a retrospective study done in November 2018. This study included patients, with cholelithiasis or choledocholithiasis diagnosed through radiological studies, in the department of general surgery in our hospital during the period of 1 year (October 2017 to October 2018). Data of all the patients diagnosed with cholelithiasis was collected from Medical Records Department. Patients who had undergone thyroid function tests during their course of admission in the hospital were noted and the values of thyroid function tests documented and analyzed. Number of patients with hypothyroidism were noted. Patients with a history of medical or surgical thyroid intervention were excluded. According to the hospital standard values, patients with thyroid stimulation hormone (TSH) between 0.35 and 4 IU/ml were considered Euthyroid, TSH >4 IU/ml as Hypothyroid and TSH <0.35 IU/ml as Hyperthyroid. Results: Age of the patients ranged from 20-70 years. 29.03% were male patients and 70.96% were female patients. The male to female ratio was 1:2.4. Females were the predominant group. Thyroid function test was performed in all 62 patients out of which, 20 patients (32.2%) were found to have hypothyroidism and the rest i.e. 42 patients were euthyroid. Of these 20 patients only 4 were known hypothyroid, rest 16 patients were newly diagnosed hypothyroid patients. Of these 20 patients, 14 patients were females rest were males. 50-70 yrs were the predominant group. More than 70% of the patients with hypothyroidism belonged to this group. Conclusion: Evaluation of thyroid profile should be a part of general workup in patients with both cholelithiasis and choledocholithiasis especially in females above the age of 40yrs and patient should be treated with appropriate thyroid medications as indicated.
