A Surgical Emergency In Adolescent Girl – Concurrent Imperforate Hymen And High Transverse Vaginal Septum With Tense Hematocolpos

Abstract
Rare Mullerian anomalies not falling into any present classification system are sometimes reported. This is one of the most infrequent anomalies of the female genital tract. A 15-year-old girl, not attained menarche, presented as severe cyclic lower abdominal and pelvic pain and mass per abdomen. After clinical and imaging examination, an imperforate hymen and a large hematocolpos along the middle part of the vagina was diagnosed. Further evaluation by MRI revealed a high vaginal cause of obstruction which cannot be differentiated as vaginal atresia or a combination of transverse vaginal septum and imperforate hymen. Incision of the imperforate hymen did not lead to drainage of blood or fluid. A complete high transverse vaginal septum in the upper third of the vagina was identified and when incised drained approximately 200 mL of a dense brownish fluid. Hysteroscopy revealed presence of normal uterus. Post-operative dilatation of vagina was done to prevent re-stenosis. This is a rare case of concurrent transverse vaginal septum and imperforate hymen without any other genitourinary anomalies in literature.
