Microbial Analysis Of The Vitreous Aspirates In Culture Proven Endophthalmitis

Abstract

Background: Acute endophthalmitis is a serious vision threatening intraocular infection involving the vitreous cavity. We report the microbiological profile of patients with infectious endophthalmitis who presented to a tertiary eye care centre in Hyderabad, Telangana. Material & Methods: A prospective analysis of 62 eyes diagnosed as endophthalmitis was performed over a period of 2 years. Aseptically collected vitreous aspirates were cultured for bacteria and fungus using standard microbiological techniques. Gram stain and KOH preparation of the specimens were also performed. Results: Forty five (72.58%) patients had post-operative, 15 (24.19%) had post-traumatic and 2 (3.33%) had endogenous endophthalmitis. Males were predominant in post-traumatic endophthalmitis and females were predominant in post-operative endophthalmitis. Significantly younger individuals constituted post-traumatic group. Vitreous culture was positive in 32 (51.6%) out of total 62 cases; 30 bacteria and 2 fungi were isolated. Gram positive bacteria (17 cases - 53.1%) were more common than gram negative (13cases – 40.6%). Two fungal cases (6.25%) were isolated in this study. All Gram-positive bacteria, except Nocardia, were susceptible to vancomycin. Gram negative bacteria showed sensitivity to amikacin and ceftazidime. Conclusion: In post-operative group, both Gram-positive and Gram-negative showed equal incidence. In post traumatic group gram positive bacteria were more commonly isolated accounting for 66.6%.
