Achievements in therapy of HIV – infection

Abstract

Even though 26 years passed since the HIV virus has been isolated, we are still not capable to eliminate the virus from the infected organism. The main aim of the therapy is then to reduce the symptoms, restrain the progress of the disease, and rebuild the HIV-positive patients immune system. Knowing the structure and etiopathogenesis of the HIV infection made us capable of synthesizing drugs, that either directly block the virus penetration to the other cells.The first drug using in therapy of HIV infection was zydovudine – reverse transcriptase inhibitor. Unfortunately, trials of zydovudine monotherapy usage failed – because of the rapidly occuring virus resistance to this drug. Not until the new drugs generations and applying the combined antiretroviral therapy (cART) it became possible to extend survival and improve life quality of the HIV positive and AIDS patients. Currently six groups (with one subgroup) of antiretroviral drugs are available including near 30 agents. The most frequently used schemes are based on the combinations of the two nucleoside reverse transcriptase inhibitors and one non-nucleoside reverse transcriptase inhibitor or protease inhibitors boosted with 100 miligrams of ritonavir. Researches of the new antiretroviral agents, therapeutic options, and the effective vaccine are in progress, giving hope for the further HIV-infected patients life comfort improvement, and possibly, finding the method of the complete virus eradication from the human body.
