Acute myeloid leukemia during pregnancy. A case report and review of the literature

Abstract

Malignant neoplasms are estimated to occur in about 1 of 1000 pregnancies and leukemias complicate as many as 1 of every 75 000-100 000 pregnancies. Among leukemias, acute myeloid leukemia predominates and due to its aggressive course requires immediate implementation of intensive cytostatic treatment, what in obstetric patients is associated with substantial therapeutic and ethical dilemmas. We report a case of a 21-year-old woman in which acute myeloid leukemia was diagnosed at 21 weeks of gestation. At the initial stage of the treatment steroidotherapy was used, with an intent to postpone induction chemotherapy until delivery. In the 29th week of gestation, deterioration of patient’s condition was observed, associated with the occurrence of pneumonia and due to this life-threatening condition Caesarean section delivery was conducted, resulting in a live birth. Then individualized cytostatic induction therapy was performed, complicated unfortunately by bilateral pneumonia and septic shock that was the cause of patient’s death. Due to the low incidence of acute leukemias during pregnancy there are still no guidelines for their management, according to which physicians should proceed. The stage of pregnancy, mother’s and child’s condition must be considered during the selection of an appropriate individualized treatment.
