Ankylosis – methods of diagnosis and treatment Literature review

Journal Title: Forum Ortodontyczne - Year 2017, Vol 13, Issue 4

Abstract

Ankylosis is a common complication after a trauma, replantation of an avulsed tooth or tooth transplantation. It is a result of replacement resorption leading to a union between root cement and dentin with the alveolar process bone. A root of such a tooth is gradually replaced by bone tissue of the alveolar process, and it may lead to complete replacement of root tissues with bone and loss of a tooth crown. Diagnostics is based on the analysis of radiological images of an examined teeth, percussion test and precision tests with Periotest or Osstell Mentor. Presence of ankylosis in children and adolescents is associated with progressive tooth infraposition, delayed growth of the process bone and inclination of adjacent teeth. In order to reduce a risk of complications it is necessary to introduce an appropriate method of treatment considering a current stage of patient’s growth and development. Aim. The paper aims to present methods to diagnose and treat ankylotic teeth. Material and methods. The paper presents characteristics of ankylosis, methods of its diagnosis and treatment based on a literature review from PubMed, US National Library of Medicine, and Digital Library of Wielkopolska. Results and conclusions. With regard to numerous methods of treatment of ankylotic teeth the method presented by Malmgren et al. that involves decoronation (amputation) of a tooth crown and leaving the root until it has become completely resorbed seems to be studied the best. With time, treatment options were perfected and expanded with the use of distractors and micro-implants used in surgical treatment aimed at preparation of the alveolar process bone for further implant-supported prosthetic restoration. With regard to many current methods of treatment of ankylotic teeth, selection of an appropriate method that is suitable to the patient’s developmental stage seems to be the most important. (Bilińska-Koczwara M, Rucińska-Grygiel B, Bielawska-Victorini H, Jagła S, Fus A.
