Application of adhesive systems in biological treatment of the pulp

Abstract

Introduction. Healthy pulp provides an effective barrier to counteract the penetration of microorganisms through the cavity in tooth to the periapical tissues and into the human body. Basic material used in the methods of direct and indirect pulp capping is calcium hydroxide. Even though it’s high biocompatibility, antibacterial, anti-inflammatory and odontropic qualities, it is not an ideal material. Hence the search for other materials for direct pulp capping for instance - adhesive systems. Aim of the study. The aim of the study was to present on the basis of the contemporary scientific literature, information on the use of adhesive systems for biological treatment of the pulp. Material and Methods. Comprehensive, systematic review of the literature published in the years 2006-2016 in the database PubMed and GBL on the use of adhesive systems for the treatment of biological pulp was conducted. Keyword used: „adhesive systems”, „biological treatment of pulp”, „endodontics.” Results. The idea the use of adhesive systems for the treatment of biological pulp is based on the fact that pulp isolation from the adverse effects of the environment of the oral cavity is sufficient to create the conditions for tissue regeneration and to form dentin bridge. Adhesive systems are scientifically proven to be cytotoxic, mutagenic and carcinogenic. Components of resin, such as HEMA, Bis - GMA, TEGDMA have inhibitory activity on the replication of DNA and protein synthesis. They also have immunosuppressive properties on the human body. Conclusions. Current research does not seem to justify routine use of adhesive systems in the process of biological treatment of pulp. The course of regeneration of the pulp is affected by many factors such as age of the patient, obstruction of pulp bleeding and degree of decontamination. Some researchers state that the right diagnosis of the pulp’s condition is the key to the success of the treatment and the materials used are of less importance.
