Application of failure mode and effects analysis to minimize quality failures in clinical biochemistry laboratory
Journal Title: International Journal of Clinical Biochemistry and Research - Year 2018, Vol 5, Issue 4
Abstract
Introduction Quality failures in the clinical laboratories should be analyzed to improve patient safety in hospital Purpose of this study is to apply failure mode and effects analysis FMEA for prospective risks of quality failures and appropriate corrective actions to reduceprevent errors in clinical biochemistry laboratoryMaterials and Methods Members of multidisciplinary team were trained to notify quality failures Each quality failures assigned value from 1 to 5 based on severity occurrence and detection of failure modes Risk priority number RPN was calculated from severity occurrence and detection scores RPN SI x OI x DI For highest risk failure modes FMEA tool was applied in two stages before and after action planResults A total 14 high risk failure modes were found and arranged based on their RPN values from high to low score In 5 highest risk failure modes RPN values before action plan were as follows Transcription error RPN100 Malfunction of reagent RPN75 Malfunction of calibrator RPN48 Samples taken in wrong tubes RPN36 and Sample misplaced in laboratory RPN36 After corrective actions taken we found decrease in RPN values for 5 highest risk failure modesConclusion FMEA is an effective tool to reduce quality failures in clinical biochemistry laboratoriesKeywords Failure mode and effects analysis FMEA Quality failure Patient safety Risk priority number RPN

Evaluation of Adenosine deaminase, Serum C-reactive protein and alkaline phosphatase activity in Rheumatoid Arthritis patients in Shivamogga district, South India
Introduction & Objectives: Rheumatoid arthritis (RA) is a prototypical inflammatory joint disease. The degree of inflammation is reflected in the extent of joint damage, which further has influence on the quality of life...
Speculation of diabetic nephropathy by pro-inflammatory cytokines (IL-6, IL-10) & renal biomolecules
Introduction: Type 2 diabetic nephropathy (T2DN) develops inflammation in kidney which leads to failure. Study designed to evaluate risk of diabetic nephropathy (DN) by inflammatory gene expressions (IL6, IL10) and renal...
Association of 25 OH-Vitamin D and hs-CRPin adults with Essential Hypertension
Essential hypertensionis a typical example of complex, multifactorial trait and a well studied risk factor for cardiovascular disease. hs-CRP is well known marker of inflammation resulting in atherosclerosis and cardiova...
Prevalence of Haemoglobinopathies in Punjabi population
Background: The prevalence of thalassemias and various hemoglobinopathies are a major public health problem in Punjabi population. These cases are mainly overlooked and thus only treated as a part of anaemia. Thus it is...
Uric acid in Type 2 Diabetes mellitus with nephropathy
Introduction: Uric acid is an important antioxidant that contributes for around 60% of free radical scavenging capacity of serum. Its role has been controversial in type 2 diabetes mellitus. Earlier studies have reported...
