Bile duct injury complicating laparoscopic cholecystectomy: risk factors and prevention

Abstract
Laparoscopic cholecystectomy is a most common performed operation in abdominal surgery. Steady progress in technical equipment used in this procedure not protects against the risk of bile duct injury yet. Problems concerning prevention of bile duct injury are discussed in this article. Among methods that minimize the risk of bile duct injury detailed Calots’s triangle visualization (critical view of safety), low threshold of conversion, avoidance of retrograde cholecystectomy, intraoperative cholangiography, and some operative maneuvers are proposed. Learning of all traps waiting on surgeon (disease, technique and anatomy) will reduce risk of bile duct injury in future. However this danger will be never completely eliminated mainly because of imperfect human eye perception of picture displayed on monitor screen.
