Clinical consequences of Bolton Index deviations

Abstract
Deviation of the proper Bolton Index has an essential clinical meaning in the case of double overdraw of the standard deviation value. In the patient with a proper occlusion and with normal position of front teeth we can observe gaps, crowding, rotations or increased overjet. Dysharmony of the teeth width between lower and upper arch can be treated by extraction, approximal enamel reduction or reconstruction of crowns shape.[b]Aim:[/b] The evaluation of treatment results in patients with large deviation of Bolton index, combined with the presentation of possible therapeutic options. [b]Subject and methods[/b]: Plaster casts, intra- and extraoral photos of 50 patients treated with fixed appliance taken before and after orthodontic treatment. In the study the patients with Bolton index deviation more than 2 mm were included. Bolton index, occlusion and overjet were evalueted at the beginning. Applied corrective therapeutic procedures were presented. [b]Results: [/b]In all treated patients the improvement of the occlusion was found after the application of the following therapeutic procedures: approximal enamel reduction in the lower arch, approximal enamel reduction in the upper arch, reconstruction of the crown shape in the upper arch, extraction of the additional incisor, space closure after missing incisor in the lower arch, restoring the space for the lower missing incisor. The majority of patients reached the correct value of OJ and OB, Class I canine relationship, the ideal occlusion on the posterior teeth and good esthe-tic result. [b]Conclusion[/b]: Orthodontic treatment in patients with Bolton index deviation seems to be difficult and requires unusual decisions. Therapeutic procedures allow us to approach the “golden standard” in patients with normal values of Bolton index. (Orthod. Forum 2011; 7: 69 -77). Received: 2.03.2011 Accepted: 13.06.2011
