Abstract
AimandBackground:AimandBackground:TheprimaryobjectiveofthisextensiveseriesistoconductacomparativeanalysisofmicroscopesandendoscopesandinvestigatetheextentofvariabilityinCSFleakagebasedonthesurgicalapproachemployed,theoccurrenceofcomplicationsduringtheprocedure,thetechniquesemployedforclosure,andanypostoperativeinterventions.Method:AretrospectiveanalysiswasconductedonlesionsinthesellarregionthatweresurgicallytreatedviathetranssphenoidalapproachintheDepartmentofNeurosurgery.Microscope,endoscope,orbothwereusedduringtheapproach.Theaccessrouteusedinthisstudywasthetransnasaltranssphenoidal(TNTS),sublabial,ortranscranial+TNTSapproach.TherepairprocedureinvolvedtheuseofaSurgicel-coatednasalflap,fatgraft,orfasciagraftandfibringlue.Results:ThecohortofindividualsexperiencingCSFleaksexhibitedanotablyelevateduseofendoscopicprocedures,whiletheuseofmicroscopictechniqueswasrelativelyfew.Theincidenceoffatgraft,nothing,fat+fascia,andthepresenceofsurgicelwasratherlowinpatientspresentingwithCSFleakage.Therewasnostatisticallysignificantdisparityobservedinthefrequenciesoflumberpunctureamongpatientswhodidnotexhibitcerebrospinalfluidleakage.TheratesoflumberdrainageandmeningitiswerefoundtobeconsiderablygreaterinpatientswithCSFleakcomparedtothosewithout(p<0.001forbothvariables).Conclusion:Ithasbeensuggestedthatselectinganappropriateclosureapproachshouldbebasedontheintraoperativecomplicationsobservedduringthesurgicalprocedure,topreventCSFleakage.
