Combined treatment of temporomandibular joint dysfunction - a case report

Abstract
Temporomandibular disorders (TMDs) that involve abnormalities in temporomandibular joints, masticatory muscles and surrounded structures, are one of the most common problems reported by patients. Due to multifactorial aetiology and a wide spectrum of concomitant signs and symptoms, the treatment of patients afflicted to these disorders needs to be combined and interdisciplinary, bringing together a diverse group of specialists. The main role of dentists is to achieve the orthopedic balance in the masticatory organ. Aim. The aim of the study was to analyse the process of treatment of a female patient with temporomandibular dysfunction, also with manifested by discrepancy between the mandibular position in maximal habitual intercuspation and the mandibular position optimal for the function of temporomandibular joints. Material and methods. The patient was diagnosed with internal derangement of TMJ in the form of joint disc displacement without reduction and osteoarthritis. After the first stage of orthoprosthodontic treatment was applied to stabilize the mandibular therapeutic position. Results. The combined treatment of temporomandibular joint dysfunction was successful and allowed the dentist to obtain the orthopedic stability in the masticatory organ. Conclusions. The combined ortho-prosthodontic treatment can prove to be an efficient method for restoring the orthopedic balance within the stomatognathic system in patients resulting in the regress of dysfunction symptoms. An analysis of occlusion should be carried out after the first phase of treatment, when it is possible to assess the mandibular position optimal for the masticatory organ function, improving the function of temporomandibular joints, which could be stabilized by teeth occlusal contacts.
