Contribution of yeast-like fungi in hospital infections

Abstract
Introduction: Despite loads of epidemiological data, hospital mycotic infections pose a not always noticed problem. Aim of the study: Determination of the contribution of yeast-like fungi in etiology of hospital infections based on mycological analyses of biological material sampled from patients as well as isolates originating from the hospital environment. Material and methods: The experimental material were yeast-like fungi isolated from patients with multifocal infections hospitalized for 4 to 6 weeks, in whom the result of a mycological test was negative on admission to hospital. Cultures and microcultures as well as fungi identification to species were conducted following routine mycological diagnostics. The taxonomic spectrum of fungi was compared with results of mycological analyses of the hospital environment. The diffusion-disk method was applied to determine drug-susceptibility of all isolates against fluconazole and nystatin. Results: A group of persons suspected of hospital infections included 54 patients (23.3%), from whom a total of 230 fungi isolates were obtained, includ- ing 180 of the genus Candida and 50 of the genus Trichosporon. The fungi isolated were predominated by C. albicans (34.8%) and T. beigelii (21.8%). C. dubliniensis (17.3%), C. tropicalis (17.3%), and C. krusei (8.7%) were identified less frequently. The first two species were also detected in sediment samples (patients' rooms, treatment rooms) and T. beigelii occurred also in catheters. Very high and high resistance to fluconazole and nystatin were demonstrated by isolates of C. krusei, C. albicans and T. beigelii. Conclusions: In 1/3 of the hospitalized patients fungi appeared during their hospital stay.The results obtained confirmed a high contribution of C. albicans in hospital infections. The second major anthropopathogen seems to be T. beigeliidisplaying ever increasing expansiveness towards organ ontocenoses, especially in closed environments.
