Correlation of cytomorphology and cartridge based nucleic acid amplification test (CBNAAT) on fine needle aspirates in the diagnosis of tuberculous lymphadenitis

Abstract
Background: As per the Global TB Report 2017, India had about a quarter of the world’s TB Cases with a very high incidence of MDR TB and HIV associated TB and high mortality; mostly because of lack of early diagnosis and treatment. In 2014, WHO recommended CBNAAT over conventional tests in patients suspected of having extra pulmonary TB for early diagnosis and treatment. Materials and Methods: A 1 year prospective study conducted between July 2017 to July 2018 at Department of Pathology, MGM Hospital/Kakatiya Medical College, Warangal on patients presumed of having tuberculosis presenting with lymphadenopathy. Results: Out of the total 73 cases, 30 showed positive TB cytomorphology, 19 were positive on AFB staining and 17 were positive on CBNAAT. Conclusion: FNAC and AFB are mutually complementary, simple and when used together offered early diagnosis of TB lymphadenopathy in low resource settings. CBNAAT may be used to diagnose Rifampicin resistance in suspected cases, immunosuppressed individuals (PLWHA) and children and should be supplemented with FNAC.
