Depression in in-patient and outpatient treatment cancer patients and influencing factors<p>Yatarak ve ayaktan kemoterapi alan kanser hastalarında depresyon ve etkileyen faktörler

Abstract
Cancer brings serious psychological and sociological problems for patients and their relatives. The most common psychopathology seen in this patient group is depression. This study is conducted with the purpose of identifying the levels of depression seen in the cancer patients and their relatives and the factors affecting it. This is a descriptive study. The universe and sample of this study is in-patient and outpatient chemotherapy patients of Bulent Ecevit University Training and Research Hospital for the period February 15 – March 15, 2012. There were 146 patients and 127 patient relatives who accepted to contribute to this study. In collection of the data, Personnel Information Form and Beck Depression Measure (BDM) have been used. The BDM scores were 36.58 ±10.01 for the patients and 30.25 ±9.65 for the patient relatives. There is no statistically meaningful difference between BDM point averages of patients who know the diagnosis and have enough information about the illness (p>0.05). There is a statistically meaningful difference between the groups and BDM point averages are higher for patient relatives who know the illness of their patients (p<0.05). As a result of this study, it is seen that, both patients and their relatives face a severe depression; being an in-patient or outpatient, knowing the Diagnosis of disease having adequate knowledge does not effect the depression points; depression points are higher for those patient relatives that know the identity of disease of their patients. ÖzetKanser, hasta ve hasta yakınları ve toplum için ciddi psikolojik-sosyal sorunları beraberinde getirmektedir. Bu hasta grubunda en sık görülen psikopatoloji ise depresyondur. Bu çalışmada kemoterapi alan hasta ve hasta yakınlarının depresyon düzeylerini ve etkileyen faktörleri belirlemek amacıyla yapılmıştır. Tanımlayıcı olarak yapılan bu çalışmanın evren ve örneklemini 15 Şubat-15 Mart 2012 tarihleri arasında Bülent Ecevit Üniversitesi Eğitim ve Araştırma Hastanesinde yatarak ve ayaktan kemoterapi alan hastalar ve hasta yakınları oluşturdu. Çalışma grubunu katılmayı kabul eden 146 hasta ve 127 hasta yakını oluşturdu. Verilerin toplanmasında Kişisel Bilgi Formu ve Beck Depresyon Ölçeği (BDÖ) kullanıldı.Hasta grubunun BDÖ puan ortalaması 36,58±10,01; hasta yakınlarının BDÖ puan ortalaması 30,25±9.65’tir. Hem hasta grubunda hem de hasta yakını grubunda ayaktan ya da yatarak tedavi görmekle BDÖ puanları arasında istatistiksel olarak anlamlı bir fark yoktur (p>0.05). Hastanın tanısını bilmesi ve hastalığı hakkında yeterli bilgiye sahip olma durumları ile BDÖ puan ortalamaları arasında istatistiksel olarak anlamlı bir fark yoktur (p>0.05). Hastasının tanısını bilen hasta yakınlarının, BDÖ puan ortalamaları yüksek ve gruplar arasındaki fark anlamlıdır (p<0.05). Bu çalışmanın sonucunda hem kemoterapi alan hastalar hem de yakınlarının şiddetli düzeyde depresyon yaşadığı; ayaktan ya da yatarak kemoterapi almanın, hastaların tanısını bilmesinin, yeterli bilgiye sahip olmasının depresyon puanlarını etkilemediği, ancak hastasının tanısını bilen hasta yakınlarının depresyon puanlarının yüksek olduğu belirlendi.
