Derivación de Rescate: una medida efectiva para reingresar al Programa Cardiovascular a pacientes hipertensos descompensados

Abstract

INTRODUCTION: Arterial hypertension has been described as one of the most important modifiable cardiovascular risks. This study communicates the experience of a regulated improvement model in communication nets between the Emergency service and the Chronic unit, to rescue a non controlled hypertense group that assist to the Emergency service. Patients and methods: Quasi-experimental like community essay in hypertensefound patients during spontaneous urgency consult was made, at the Emergency Service in Coinco’s Hospital, VI region, Chile. The data from attended patients in October 2005 was compared, and after three months of development of the rescue derivation program, in October 2006. Uncompensated, unassistant, or passive patients were entered to the program. Data was analyzed with Microsoft Excel and Epidat 3.1 software. Results: In 2005, a 30, 4% of patients were passive or unassistant regarding to their controls, in difference to 2006, where this group was a 16,7%, but this difference was not significant. Discussion: a lower acute descompensation rate was observed in hypertensive patients that were under control, also a lower percentage of unassistant and passive patients. To have a rescue derivation program from the emergency service allowed to increase the real coverage of the cardiovascular program by diagnosing newly hypertensive patients.
