Effects of nicotinic acid on phosphorus-calcium metabolism in patients with diabetic nephropathy

Abstract
Objective — to study the effects of therapy with the use of nicotinic acid on the clinical and laboratory parameters and the levels of FGF23, calcium and phosphorus in the serum of patients with diabetic nephropathy (DN). Materials and methods. In the dynamics of treatment, the examinations have been performed on 54 patients with type 2 diabetes mellitus, aged 46 to 78 years, with clinically significant nephropathy — proteinuria and decrease of the glomerular filtration rate. All patients received standard basic hypoglycemic and nephroprotective therapy. Patients with impaired calciumphosphorus metabolism were divided into the following groups: patients who received basic therapy and patients who were prescribed nicotinic acid in addition to basic therapy. The FGF23 levels in serum were determined by ELISA. Calcium and phosphorus serum concentrations were studied using a reagent kit from Cormay (Poland). Results and discussion. In patients with DN, impaired phosphoruscalcium metabolism is characterized by an increase in serum FGF23 and hyperphosphatemia. Combined therapy using nicotinic acid, as well as basic therapy, contributed to a significant decrease in the serum FGF23 content in patients. The additional administration of nicotinic acid, in contrast to the basic therapy, led to a significant decrease in phosphorus levels by 30 % (p < 0.05). Patients with DN which use basic therapy, a significant decrease in total cholesterol by 25.1 %, triglycerides by 42.7 % was found, in the group of patients, nicotinic acid was additionally obtained, a more significant decrease in these indicators was found, the level of total cholesterol decreased by 32.3 %, triglycerides by 51.4 %. Conclusions. Our findings suggest that direct effect of nephroprotective therapy on the content of circulating FGF23 and phosphorus levels in the blood serum and on the feasibility of using nicotinic acid to improve the cardionephroprotective strategy in the treatment of patients with DN.
