Efficacy of triple test for the diagnosis of palpable breast lump

Abstract
Background: Breast disorders are common and most of the complaints referred to breast not cancer related; benign diseases are more frequent. Medical history, physical examination mammography, thermography, ultrasonography and cytological examination of aspirates of nipple discharge are useful tools for diagnosis of breast lesions. We evaluated efficacy of triple test (combined medical examination, mammography, FNAC) for the diagnosis of palpable breast lump. As these tests are minimally invasive with less side effects, evaluation of combined efficacy for diagnosis of breast lump will be useful for early diagnosis. Material and Methods: Patients admitted in surgical ward presenting with breast lump constituted the material for this study. After consent these patients were submitted to three diagnostic procedures of clinical examination, mammography and FNAC. Results: In the present study, the overall correlation of clinical, mammography and FNAC findings with histopathological diagnosis were in 57 cases (76 %) out of which 33 (44 %) cases had malignant lesion and 24 cases ( 32 %) had benign lesion. The clinical findings and histological diagnosis was correlated in 70 cases (93.33 %) while mammographic finding and histopathological diagnosis was correlated in 57 cases (76 %), FNAC diagnosis and histological diagnosis was correlated in 73 cases (98.66 %). Conclusion: If all the three method diagnosis is malignant lesion, the treatment can be started without surgical biopsy. If the result of all the three procedures are suggestive of benign pathology then a wait and watch attitude is acceptable with due provision for regular follow up. In the event of doubtful or divergent results with all three technique the nature of lesions must be ascertained by histopathology. The combined sensitivity of physical examination and FNAC is more as compared to mammography and therefore mammography may be avoided where it is not available.
