Ethical Reflections on HIV Cure Research: Values and Challenges

Abstract
The limitations of current antiretroviral therapy have accelerated global efforts to explore HIV cure strategies; however, the rapid advancement in this field has also raised complex ethical issues.The article first clarifies the concepts of remission and cure, as well as sterilizing cure and functional cure. It then describes the unique value of HIV cure research. At the individual level, it can free people from continuous treatment burden and stigma. At the public health level, it can fundamentally reduce transmission risks, provide long-term socioeconomic benefits, and alleviate the strain on healthcare resources. Finally, the article provides an in-depth analysis of the ethical challenges and strategies encountered in the research process, including epistemological differences regarding cure, the "cure misconception", fair selection of research participants, and the implementation challenges of ethical review. Through forward-looking reflections, the article aims to provide insights and recommendations to support the ethical and beneficial development of HIV cure research.
