Evaluation of Conversion Rate (Operative) In Patients of Blunt Abdominal Trauma Managed Conservatively (Non- 0peratively): An Observational Study of 30 Cases

Abstract

BACKGROUND: In view of increased morbidity arising out of overzealous surgical intervention with minor survival benefit recent trends have shifted towards conservative management of blunt abdominal trauma not only type and grade of injury but mode , type and duration of injury are also accounting factors of successful outcome. MATERIAL AND METHODS: In this study 30 cases of blunt abdominal trauma with solid organ injury with or without other associated injuries (excluding visceral perforation) who were haemodynamically stable initially or after successful resuscitation were studied which were admitted in new civil hospital, Surat in different surgical units between March 2015 to November 2015. RESULTS: Out of 30 cases of blunt abdominal trauma selected for conservative management liver was most common organ found to be injured closely followed by spleen and conversion to operative management was required in 1 patient (3.3%) of splenic injury following persistent clinical deterioration. Hospital stay for patients managed conservatively is significantly less than patients managed operatively. CONCLUSION: Non operative management has promising successful results in an institute with good diagnostic, patient monitoring and treatment facilities with a conversion rate of 3% in present study. Clinical and hemodynamic stability are responsible for success of conservative management. Improved patient are brought to hospitals in less period of time and could be successfully resuscitated.
