Dexamethasone Cyclophosphamide Pulse therapy in Pemphigus Vulgaris; a case study of 25 cases

Abstract
BACKGROUND: Dexamethasone Cyclophosphamide Pulse therapy commonly known as DCP Therapy was introduced by Pasricha et al in 1982 in India for treatment of Pemphigus Vulgaris. Since then its use is extensively studied for long term remission and side effects in pemphigus vulgaris and other immune-bullous disorders. OBJECTIVE: This retrospective study is done to ascertain the role of DCP therapy, its efficacy and side effects in a tertiary care government hospital of Saurashtra region in patients of Pemphigus Vulgaris. METHODS: 25 patients of Pemphigus Vulgaris treated with DCP therapy every 28 days over the period of 5 years (January 2011 to December 2015) were studied. Patients were also given parenteral antibiotics to prevent infection and good nursing care and symptomatic treatment for better healing of the lesions. RESULTS: All 22 patients who completed the study showed complete remission. On average 3-4 pulses were required in phase I to achieve remission. Patients were given another 6 pulses of DCP in phase II. Maximum duration of remission was 3 years. None of the patient discontinued the study or died because of side effects. CONCLUSION: DCP therapy has shown promising results in terms of long term remission and minimal side effects.
