Evaluation of kidney function in diabetic patients undergoing percutaneous coronary interventions

Abstract

<b>Aim. </b>Increasing prevalence of chronic kidney disease (CKD) is mainly due to aging process, epidemic of diabetes and late diagnosis of asymptomatic early stages of CKD. Individuals with a reduced glomerular filtration rate (GFR) below 60 ml/min are at greater risk for complications after percutaneous coronary interventions, therefore the aim of the study was to assess kidney function in diabetic and non-diabetic group of patients referred for PCI and to estimate incidence of CKD in both populations.<br><b>Material and methods.</b> The study was performed on 185 patients with ischemic heart disease, referred for coronary angiography, categorized into two groups: the first group consisted of 121, the second group consisted of 64 non diabetic patients. Glomerular filtration rate was assessed using 4 formulas: Cockcroft-Gault equation, lean body mass-adjusted Cockcroft-Gault equation, simplified MDRD equation and CKD-EPI study equation.<br><b>Results. </b>In presented study in both patient groups the values of mean serum creatinine concentration were within reference ranges. However after the assessment of GFR using simplified MDRD equation it appeared that 30% of diabetic patients and 20% of non-diabetic patients can be diagnosed with chronic kidney disease. Nevertheless, the presence of chronic kidney disease defined as GFR below 60 ml/min/1.73 m<sup>2</sup> changed, for example in diabetic group from 25% using Cockcroft-Gault equation to 48% adjusting lean body mass to this formula.<br><b>Conclusions.</b> The incidence of CKD (defined as GFR below 60 ml/min) in diabetic patients referred for percutaneous coronary interventions is high despite normal creatinine concentration.
