Evaluation of mannheim peritonitis index prognostic scoring system in perforative peritonitis in Indian setup

Abstract
Background: Perforation peritonitis is one of the most common problems in general surgery practice with high mortality rate. Early prognostic evaluation of peritonitis is desirable to select high risk patient for more aggressive therapeutic procedure and planned relaparotomy.The Mannheim Peritonitis Index (MPI) has emerged as a reliable marker for accessing severity and prognosis of intra-abdominal infection. Aim: To evaluate MPI scoring system in defining the prognosis of the patients as well as for better patient care and management. Material and Methods: Total 50 cases with a clinical diagnosis of secondary peritonitis irrespective of age and sex group were considered for this study. Operation were performed through a midline exploratory laparotomy incision. Outcome of peritonitis were predicted by MPI. Results: Most patients were between 21-30 years of age.A total of 45 patients recovered and 5 died. The patients who survived had a mean MPI score of 17.91 and those who died had a mean MPI score of 28.20. MPI was analysed and was roven to be a good indicator of prognosis with a sensitivity of 80%, specificity of 97% and accuracy of 96%. Conclusion:MPI is a simple tool, easy to calculate and helps in assessing the mortality and morbidity. It can be very useful tool in countries such as India where there is limitation of resources and lack of ICU facilities.
