Experimental study on the application of injectable concentrated growth factors gel in dental pulp regeneration of permanent teeth

Abstract

Objective To investigate the effect of injectable concentrated growth factors (CGF) gel on dental pulp regeneration of permanent teeth in beagles and the balance between new tissue mineralization and pulp-like tissue formation in pulp regeneration. Methods The pulp infection model of beagles was established firstly. Then, the pulp tissue was removed and the root canal was prepared. The anterior teeth of beagles were randomly divided into four groups: empty canal group (only removal of the pulp tissue), revascularization group (stimulation of apical bleeding to fill the root canal), CGF group (injection of CGF gel into the root canal to fill the root canal), and revascularization + CGF group (stimulation of apical bleeding to fill the root canal by 1 / 2, injection of CGF gel into the root canal by full length). The root canal was closed with iRoot-BP Plus in each group, and the crown was filled with flow resin. X-radiographs were taken before surgery and 3 months after surgery to observe periapical image changes, and histological observation was made 3 months after surgery. Results X-radiographs showed that no obvious apical shadow was observed before and after pulp regeneration in each group, but blurred hard plate of periapical bone was observed in the empty canal group. HE staining showed that no significant new tissue was observed in the root canals of the empty canal group. In the revascularization group, the new tissue reached about 1/ 2 of the root canal, and was mainly bone-like tissue. In the CGF group, the new tissue in the root canal was mainly loose connective tissue with vascular structure, which could reach about 2 / 3 of the root canal. In the revascularization + CGF group, the new loose connective tissue in the root canal was similar to the healthy pulp tissue, with multicellular layer and cell-free layer, and the vascular structure was evenly distributed. A mineralization bridge was formed in the crown part of the root canal which wrapped the new pulp tissue. In addition to the empty canal group, neonatal dentin-like tissue could be seen in the root canal wall of the other three groups. Conclusion Injectable CGF gel can promote pulp tissue regeneration, and its combination with revascularization can regulate the balance between new tissue mineralization and pulp tissue formation.
