Functional Outcome Of Displaced Intra-Articular Fractures Of The Calcaneus Treated With Minimally Invasive Fixation: A Retrospective Study

Abstract
Background: Management of displaced intra-articular fractures of the calcaneus remains a challenge despite recent advances in diagnostic and operative techniques. Traditional open reduction and internal fixation is associated with high incidence of soft tissue complications. Minimal invasive operative techniques of selected intra-articular fractures offer a balance between acceptable reduction of the fracture and minimisation of soft tissue complications. Materials and Methods: We performed a retrospective study of 31 patients (32 fractures) with Sanders type 2 and type 3 and Essex-Lopresti joint depression and tongue type intra-articular fractures who presented to our institution in the period from January 2010 and December 2014. Closed reduction and percutaneous fixation with cannulated screws were done in all cases. Functional outcome was evaluated in all patients at 1 year follow up using the AOFAS hind foot score. Radiograph taken at 1 year follow up was compared with that of the uninjured side. Results: Using the AOFAS hind foot score, 6 patients (19.3%) had excellent results, 17 patients (54.8%) had good results, 5 patients (16.1%) had fair resultsand 3 (9.8%) patients had poor results. Satisfactory results, i.e. excellent and good results, were seen in 74.1% of the cases. The calcaneal morphology was restored to near normal in 27 (87%) cases. Conclusion: We conclude that closed reduction and percutaneous fixation of selected intra-articular calcaneal fractures result in acceptable fracture reduction and has good functional outcome with minimal soft tissue complications.
