HE4 concentrations in healthy pregnant women and in pregnant women with pre-eclampsia – a pilot study

Abstract

Aim. The main goal of the study was to assess third trimester and postpartum HE4 concentrations in healthy pregnant women and in pregnant women with pre-eclampsia in analogous gestational age. It was also aimed to compare the findings to the results in non-pregnant women. Material and methods. The prospective study included in total 102 women with a single pregnancy and 50 non-pregnant women. The pregnant women were divided into two groups: 50 with pre-eclampsia and 52 in healthy pregnancy. HE4 concentrations were assayed twice in pregnant participants (in the third trimester and directly after childbirth) and once in non-pregnant participants using Human HE4 PicoKine™ ELISA Kit. Results. The third trimester HE4 levels were higher in women with pre-eclampsia (p<0.0001) and in healthy pregnant women (p<0,0001) than in non-pregnant participants. The same was observed for assays made directly after childbirth (p=0.0007 and p<0.0001, respectively). The postpartum HE4 levels were lower in women with pre-eclampsia than in healthy pregnant women (p<0.0001). The postpartum HE4 levels were higher that the third trimester levels in both healthy pregnant women (p<0.0001) and in women with pre-eclampsia (p<0,0003). In pre-eclampsia patients, the third trimester HE4 levels show a significant negative correlation with neonatal body weight (R=-0.4439) and Apgar score at 1 minute (R=-0.3911), and the postpartum levels correlate negatively with Apgar score at 1 minute (R=-0.7134). Conclusions. Childbirth is a potent stimulus of HE4 synthesis. Pre-eclampsia is not a sufficient factor to induce additional HE4 synthesis. The negative effect on birth weight and Apgar score at 1 minute in the pre-eclampsia group may indicate potential relevance of the HE4 protein in this group of patients.
