Histopathological changes in coronaries and thoracic aorta

Abstract

Background: Atherosclerosis is characterized by intimal lesions called atheromas or atheromatous fibro fatty plaques, which protrude into and obstruct vascular lumen and weaken the underlying media. It is a disease of elastic arteries and large and medium sized muscular arteries. The basic lesion is the atheroma/ atheromatous plaque consisting of raised focal lesion initiating within the intima having a soft yellow core of lipid, covered by a firm white fibrous cap. Methodology: The present study was carried out in Karnataka. All the autopsy specimens of heart received in the Department of Pathology for histopathological examination during the study period were included irrespective of cause of death. All the fetal and perinatal autopsy cases were excluded from the study. Results: Sudden death was mode of death in coronary atherosclerosis seen predominantly in 22 cases (53.65%) followed by found dead in 8 (19.51%), snake bite in 4 (9.75%), poisoning in 2 (4.8%), pulmonary embolism in 1 (2.4%), septic shock in 1 (2.4%), burns in 1 (2.4%), electrocution in 1 (2.4%), RTA/trauma/assault in 1 (2.4%). Conclusion: Combined involvement of right coronary artery, left circumflex and left anterior descending artery was seen predominantly in 16 cases (39.02%), followed by combined involvement of left circumflex and left anterior descending artery in 14 (34.14%), combined right coronary and left anterior descending artery in 2 (4.87%), right coronary and left circumflex in 2 (4.87%), left anterior descending alone in 4 (9.75%), left circumflex alone in 3 (7.30%).
