Intramedullary fixation in the treatment multi-level<br /> and comminuted tibia fractures

Abstract

Fractures within crus may happen among patients of all ages and they are one of the most common fractures. Taking into consideration the structure of tibia, one of the strongest bones in human body, fractures are a result of direct trauma and are related to developments in road transportation. The forces necessary for fracturing of tibia are very high and usually this type of trauma is connected with massive soft tissue damage, which presents even more difficult clinical problem. In our clinic we treat around 380 patients/year with fractures within a crus. Almost 1/3 of them are treated with closed reduction followed by fixation with intramedullary nail. Fixation of fractures with external stabilizer or plate dominates in fractures of proximal or distal end of tibia.In this paper we show the application of intramedullary nailing in the treatment of multi-level and comminuted fractures of tibia. We also present data on implementation of intramedullary nails in multistage fixation of fractures and re-fixations of complicated fractures. We demonstrate, that intramedullary fixation of tibial fracture is safe method. It allow for quick recovery and fast introduction of physical therapy. It also gives low number of complications, usually connected to device failure.
