Depression in old age – archetype of affective disorders?

Abstract
Late-life depressions are heterogenous. The relationship between cardiovascular risk factors and depression is well-known,but during last years is analyzed carefully. There is presumption,that knowledge about pathomechanisms of depression in the elderly, especially vascular depression,may elucidate mechanisms of all depressions. Definition of vascular depression is not very clear,but there are many evidence, showing different clinical characteristics, drug-resistance, pathomechanisms and social functioning of ill persons. It is presumed,that basic reason for vascular depression is dysfunction of fronto-striatal areas,and dysfunctional coordination of limbic-cortical pathways. The most important clinical symptom seems to be executive dysfunction,which is responsible for further depressive symptoms. Treatment of vascular depression, although difficult, is typical, especially with SSRI’s. There are new ideas to use other dopaminergic drugs,which are shown to be effective.
